PHASE 3
Engagement in a variety of child and adult led activities underpinned by EHCP outcomes and targets.
PHASE 2
Mentor and child/young person relationship building using a child led approach. Regular supervisions and MDT meetings held by STET consultants.
PHASE 1
Referral, initial meeting with STET consultants, mentor matching, documentation sharing, timetable and target setting. Mentor and family introduction.
